Appears to be a very old early version of the L&T Dry, single disc.  Looks like it is falling apart and could be dangerous if the weight plates are coming loose and could fly off. I would not run the engine before servicing the clutch to fix the problems.
Here’s a link to the L&T web site. Get in touch with them and send them photos to see if they still make parts to fit yours.
